How to create a patient portal account

​

1. Call 413-785-5321, or give the front desk your email at your next appointment

2. Check your email inbox for your patient portal invitation 

3. Use the temporary password provided in the email to login

4. You will then be prompted to put in a password of your choice

5. Start viewing your chart and communicating with our office right away!
