Preparing for your first visit
The Urology Group of Western New England is happy to accept new patients for treatment of all urologic conditions.
New Patients should plan to arrive approximately 15 minutes early for their first appointment. To better understand your current healthcare needs, UGWNE needs a variety of forms completed prior to your first visit. These forms help us to understand more about your current and past medical condition(s). Please complete the following forms prior to arriving for your appointment:
Please bring all insurance cards, a photo ID, and an up to date medication list to your first appointment. All medical records pertaining to your visit must be faxed or brought to our office prior to the day of the visit. If applicable, please bring a disc of any recent imaging. Be prepared to share your medical history including past and current medical conditions, past surgeries, allergies, etc.
